How do I get a copy of my medical records?

Baptist Health provides patients with several ways to request medical records:

MyChart Portal

The MyChart portal offers patients the ability to securely view and print/download portions of their medical record.  The use of MyChart portal is FREE. 
Your information is securely stored on MyChart indefinitely.

Visit or Call a Hospital

The Health Information Management Department (aka Medical Records Department) can assist you with obtaining a copy of your record.  Please be ready to provide photo ID when requesting records.

If you need records from a Baptist Health clinic, please contact the clinic directly or access the clinic patient portal

The Health Information Management Department will verify the patient’s identity and ask the patient to complete a form.  Patients can request their records in paper or electronic format.  

Receiving Records by Email

One option for electronic format is unsecured email.  The total file size must be less than 8mb.  Only text documents are eligible for email.  Image files (pictures, xray images, etc) are too large and must be provided on CD.  

When requesting/receiving records by unsecured email, patients should be aware of the following potential risks:

    • The information may be obtained by someone else
    • The information can be opened and read by someone else
    • Unencrypted information does not provide any assurance of privacy or security

Fees for Medical Records

The use of MyChart portal is FREE.   See information above.

Electronic to electronic (CD/USB/Portal):

    • $6.50 flat fee, plus sales tax as applicable

Hybrid (paper AND electronic) to electronic:

    • $6.50 flat fee for the portion of the record maintained electronically
    • Plus $2.00 representing our average labor cost to create and deliver the portion of the record maintained in paper for a standard request, plus sales tax as applicable

Paper to electronic:

    • $2.00 representing our average labor cost to create and deliver the record for a standard request
    • Plus actual postage if mailed, plus sales tax as applicable

Electronic to paper or paper to paper:

    • $2.00 representing our average labor cost to create and deliver the record for a standard request plus
    • Plus $0.05 per page for supplies (paper and toner)
    • Plus actual postage if mailed, plus sales tax as applicable

It is Baptist Health’s policy to complete requests in the order in which they are received. All requests are processed in a timely manner; however, certain circumstances may cause delays in processing. Records dated prior to April of 1999 are stored on micro-film and can take up to 30 days to process. In addition, Baptist Health allows physicians 30 days from date of discharge to complete chart information on their patients.
