UAMS and Baptist Health Medical Center-Conway have teamed up to provide complete orthopaedic services at UAMS Baptist Health Orthopaedic Clinic-Conway. The providers at UAMS Baptist Health Orthopaedic Clinic-Conway utilize state-of-the-art orthopaedic treatments and advanced surgical technology to assist patients with injury related to trauma, spine, hand, foot, fracture care, joint replacement, sports medicine, and more.

Our clinic gladly accepts referrals from all primary care facilities, and we welcome the opportunity to provide a personalized approach to your repair and recovery journey.
 

Patient Information

UAMS Baptist Health Orthopaedic Clinic in Conway, AR, welcomes you to our practice. Once you have made your initial appointment, please read through the information below to help streamline your first visit with us.

Established patients may sign in to our MyChart patient portal to schedule an appointment, request an appointment, or cancel an upcoming visit with their orthopaedic provider. Before your scheduled visit, you can use eCheck-In to update insurance information, medications, allergies, fill out questionnaires, and sign documents.

 

What can I expect at my first visit?

Our office is located on the Medical Office Building’s third floor, directly behind Baptist Health Medical Center in Conway.

Upon arrival, you will be greeted by a clinic coordinator. They will help ensure you have completed any necessary patient forms and that we have the needed copies of your insurance card and driver’s license for verification. Once your insurance is verified, the clinic coordinator will collect a copayment at this time, if applicable. 

If you have been referred to us or have not previously been seen by any of the providers in our clinic, you will need to read, complete, and submit the following information when you arrive:

  • New Patient Registration
    • Referring physical information
    • Workers Compensation Claim Information (if applicable)
    • Motor Vehicle Claim Information (if applicable)
  • Medical History
  • Notice of Privacy Practices (HIPAA)

 

What should I bring?

When you first arrive at our office, please check-in at the front desk and bring the following items with you:

  • Health insurance card 
  • List of current medications
  • Driver’s license or photo ID
  • Referral form(s)
  • Imaging (x-rays, MRIs, CT scans, etc.) for this condition
  • Workers Compensation Claim Information (if applicable)
  • Motor Vehicle Claim Information (if applicable)

 

Directions

From Interstate 40 W: Merge onto I-40 W. Take exit 129A-B for US-65 BUS W toward AR-60 W. Merge onto US-65 BUS N/E Dave Ward Dr. Turn left onto Exchange Ave. Turn right.  Your destination will be on the left.

From Interstate 40 E: Head south on I-40 E. Take exit 129 for US-65 BUS/AR-286/Dave Ward Drive W toward AR-60. Use the 2nd from the right lane to turn right onto AR-286 W/E Dave Ward Dr. Turn left onto Exchange Ave. Turn right. Your destination will be on the left.
