Greenbrier Family Clinic is a group of board-certified physicians who have been caring for Greenbrier and the surrounding communities’ families for over 25 years.
At Greenbrier Family Clinic, we evaluate and treat patients with a wide variety of acute injuries and illnesses, chronic conditions, and much more so that our patients can maintain an excellent quality of life. Prevention is a big part of our family medicine services and includes physicals, wellness exams, immunizations, and health assessments.
In addition to our general medical care, we routinely give back to our community by offering free-of-charge physical examinations to over 600 student-athletes every year.

Patient Information
Once you have made your initial appointment, please read through the information below to help streamline your first visit with us.
Established patients may sign in to our MyChart patient portal to schedule an appointment, request an appointment, or cancel an upcoming visit with your primary care provider. Before your scheduled visit with your provider, you can use eCheck-In to update insurance information, medications, allergies, fill out questionnaires, and sign documents.
What can I expect at my first visit?
Upon arrival, you will be greeted by a friendly member of our staff. They will help ensure you have completed any necessary patient forms and that we have the needed copies of your insurance card and driver’s license for verification. Once you have completed the check-in process, you will be asked to wait in our spacious waiting area until called by a nurse for your appointment.
The first visit with a provider is typically longer. We use this appointment to review your medical history and learn how to best help with your medical care.
What should I bring?
When you first arrive at our office, please check-in at the front desk and bring the following items with you:
- New patient information form
- Driver’s license or photo ID
- Insurance card
- List of medications you are currently taking
- Recent labs (within the last year)
- Imaging (most recent reports: colonoscopies, mammograms, chest X-rays, etc.)
- List of all providers you have seen in the last five years
- We need specialists’ names and phone numbers so we can request records.
Directions
From AR-25 S: Head southwest on AR-25 S towards Breaux Ln. Take a slight left onto US-65 S. Your destination will be on the right.
From US-65 S: Head south on US-65S/N Broadview St/N State Hwy 65 towards Pontiac Ln. Continue to follow US-65 S. Your destination will be on the right.
From US-65 N: Head north on US-65 N toward Beaverfork Rd. Continue on for 7.1 miles. Your destination will be on the left.
