About Beebe Family Clinic- A Baptist Health Affiliate

Beebe Family Clinic is a premier facility committed to providing top-quality care close to home for children, adults, and seniors in our community.

At Beebe Family Clinic, we understand that primary care providers are an essential part of healthcare. That is why our providers specialize in diagnosing, treating, and preventing a wide variety of acute and chronic conditions. We offer general wellness exams, immunizations, gynecologic exams, well-child visits – plus a whole lot more.

We are more than a business. We are a healing ministry, and we want to improve our patients’ health by changing the way health care is delivered. Let us get to know your history and your family’s history so that we can identify and treat minor problems before they become major ones.

Patient Information

Once you have made your initial appointment, please read through the information below to help streamline your first visit with us.

Established patients may sign in to our MyChart patient portal to schedule an appointment, request an appointment, or cancel an upcoming visit with your primary care provider. Before your scheduled visit with your provider, you can use eCheck-In to update insurance information, medications, allergies, fill out questionnaires, and sign documents.

What can I expect at my first visit?

Upon arrival, you will be greeted by a clinic coordinator. They will help ensure you have completed any necessary patient forms and that we have the needed copies of your insurance card and driver’s license for verification. Once your insurance is verified, the clinic coordinator will collect copayment, if applicable.

The first visit with a provider is typically longer. We use this appointment to review your medical history and learn how to best help with your medical care.

What should I bring?

  • Recent labs (within the last year)
  • Imaging (most recent reports: colonoscopies, mammograms, chest X-rays, etc.)
  • List of all providers you have seen in the last five years
    • We need specialists’ names and phone numbers so we can request records.
  • List of medications you are currently taking
  • Insurance card
  • Driver’s license or photo ID
  • New patient information form
