Prevention plays a significant role in our family medical services and includes physicals, immunizations, health assessments, and more.

Women’s Health

Each new phase of a woman’s life can bring different health concerns, and our providers at Baptist Health Family Clinic-Malvern are pleased to offer quality care for women of all ages. Baptist Health Family Clinic-Malvern welcomes you to our practice. Once you have made your initial appointment, please read through the information below to help streamline your first visit with us.

If you are an established patient, you may sign in to our MyChart patient portal to schedule an appointment, request an appointment, or cancel an upcoming visit with your primary care provider. Before a planned visit with your provider, you can use eCheck-In to update insurance information, medications, allergies, fill out questionnaires, and sign documents.

What can I expect at my first visit?

Our clinic is conveniently located close on the main street in Malvern.

Upon arrival, you will be warmly greeted by our front office staff. They will help ensure you have completed any necessary patient forms and that we have the needed copies of your insurance card and driver’s license for verification. Once your insurance is verified, the staff will collect it at this time if there is a copayment. 

Along with our friendly front office staff, you may also encounter physicians, nurses, lab technicians, X-ray technicians, an allergy specialist, and a dedicated care coordinator.

The first visit with a provider is typically longer. We use this appointment to review your medical history and learn how to best help with your medical care.

What should I bring?

When you first arrive at our office, please check in at the front desk and bring the following items with you:

  • Insurance card
  • Driver’s license or photo ID
  • List of medications you are currently taking
  • Recent labs (within the last year)
  • Imaging (most recent reports: colonoscopies, mammograms, chest X-rays, etc.)
  • List of all providers you have seen in the last five years
    • We need specialists’ names and phone numbers so we can request records.

 

Directions

From Interstate 30 E: Head northeast on I-30 E. Take exit 97 for AR-84 toward AR-171. Keep right at the fork and merge onto AR-84 E/Tanner St. Turn right onto US-270 BUS E. Continue onto S. Main St. Your destination will be on the right. 

From Interstate 30 W: Head southwest on I-30 W. Take exit 98A for U.S. 270 BUS toward Malvern. Merge onto US-270 BUS E/Martin Luther King Rd. Continue onto S. Main St. Your destination will be on the right.
