Baptist Health Family Clinic-DeWitt-A Service of Baptist Health Medical Center-Stuttgart connects you with the quality care you need, for whatever stage of life you’re in. We provide high-quality care to the families of DeWitt and surrounding communities.

We evaluate and treat patients with a wide variety of acute injuries and illnesses, such as strep throat or minor wounds that require care. We also treat patients with chronic conditions, such as diabetes or asthma, so they can manage their health better, remain symptom-free and maintain an excellent quality of life.

Prevention is a big part of our family medicine services and includes physicals, women’s health, immunizations and health assessments.

Reach Out and Read

Baptist Health supports early childhood education. We know that creating a solid foundation leads to healthier amazing kids, and the program is a fun experience for everyone. Dr. Julie Gardner has established the first Reach Out and Read program in southeast Arkansas. Reach Out and Read is a national non-profit program designed to help develop speech and language skills for children beginning in the newborn period and extending until they enter school around kindergarten and first grade. The purpose is to prepare them for school and set them up for success.

Scheduling an Appointment

Baptist Health Family Clinic-DeWitt welcomes you to our practice. Once you have made your initial appointment, please read through the information below to help streamline your first visit with us.

Established patients may sign in to our MyChart patient portal to schedule an appointment, request an appointment, or cancel an upcoming visit with your primary care provider. Before your scheduled visit with your provider, you can use eCheck-In to update insurance information, medications, allergies, fill out questionnaires, and sign documents.

What To Expect

Upon arrival, you will be greeted by a member of our clinic staff. They will help ensure you have completed any necessary patient forms and that we have the needed copies of your insurance card and driver’s license for verification. Once your insurance is verified, the clinic coordinator will collect it at this time if there is a copayment.

The first visit with a provider is typically longer. We use this appointment to review your medical history and learn how to best help with your medical care.

When you first arrive at our office, please check-in at the front desk and bring the following items with you:

Recent labs (within the last year)
Imaging (most recent reports: colonoscopies, mammograms, chest X-rays, etc.)
List of all providers you have seen in the last five years
We need specialists’ names and phone numbers so we can request records.
List of medications you are currently taking
Insurance card
Driver’s license or photo ID
New patient information form

Please use the designated parking at Baptist Health Family Clinic-DeWitt. We are conveniently located off South Whitehead Drive in the DeWitt Medical Arts Building.
