Once you have made your initial appointment, please read through the information below to help streamline your first visit with us.
Established patients may sign in to our MyChart patient portal to schedule an appointment, request an appointment, or cancel an upcoming visit with their primary care provider. Before your scheduled visit with your provider, you can use eCheck-In to update insurance information, medications, allergies, fill out questionnaires, and sign documents.
What can I expect at my first visit?
Our office is conveniently located just off Highway 89 next door to US Pizza and across the street from Colton’s Steakhouse.
Upon arrival, you will be greeted by an office representative. They will help ensure you have completed any necessary patient forms and that we have the needed copies of your insurance card and driver’s license for verification. Once your insurance is verified, an office representative will collect a copayment at this time, if applicable. Please arrive 30 minutes prior to your appointment time.
Though the first visit with a provider is typically longer, we work hard so that every patient is seen and treated by our provider in a professional and timely manner. We use this appointment to review your medical history and learn how to best help with your medical care.
What should I bring?
When you first arrive at our office, please check-in at the front desk and bring the following items with you:
- Health Insurance card
- Driver’s license or photo ID
- List of medications you are currently taking
