IOP-Transitions

IOP-Transitions provides outpatient counseling services designed especially for senior adults. Our program offers services during the day which allows patients to return to their homes in the afternoon. We provide group therapy sessions that incorporate fun and friendliness while discussing life’s victories and challenges. A relaxed, comfortable environment is provided where patients are encouraged to achieve individualized treatment goals.

Who We Serve

Our program is designed for the senior population, approximately 55 years or older. While dealing with change is difficult at any age, it can be especially challenging for older adults. Our program is geared toward helping manage the major life changes often faced by seniors, and improving their quality of life through the process. Participants may be dealing with the death of a spouse, loss of mobility or independence, experiencing feelings of isolation/lonliness or adjusting to new living situations. Our group activities allow you to connect with others experiencing similar challenges.

All participants are served a warm, delicious lunch and we also provide snack breaks between sessions. Services are approved and monitored by a physician.

Program Features

Our compassionate, professional staff includes a psychiatrist, registered nurse, mental health tech, 2 licensed therapists, and a certified dementia practitioner (CDP). We would like to help you or your loved one improve overall enjoyment in life. Together we will explore our interests, make new connections and bring further value and purpose to our daily journey.
