What can I expect at my first visit?
Our office is conveniently located on the campus.
Upon arrival, you will be greeted by a clinic coordinator. They will help ensure you have completed any necessary patient forms and that we have the needed copies of your insurance card and driver’s license for verification. Once your insurance is verified, the clinic coordinator will collect a copayment at this time, if applicable.
What should I bring?
When you first arrive at our office, please check-in at the front desk and bring the following items with you:
- Driver’s license or photo ID
- Health insurance card
- List of current medications
- Referral form(s)
- Imaging (x-rays, MRIs, CT scans, etc.) for this condition
