Patient Information

Baptist Health Family Clinic-White Hall welcomes you to our practice. Once you have made your initial appointment, please read through the information below to help streamline your first visit with us.

If you are an established patient, you may sign in to our MyChart patient portal to schedule an appointment, request an appointment, or cancel an upcoming visit with your primary care provider. Before a planned visit with your provider, you can use eCheck-In to update insurance information, medications, allergies, fill out questionnaires, and sign documents.

What can I expect at my first visit?

Our office is conveniently located in the heart of White Hall, next to the post office and across the street from Larry’s Pizza.

Upon arrival, you will be greeted by a clinic coordinator. They will help ensure you have completed any necessary patient forms and that we have the needed copies of your insurance card and driver’s license for verification. Once your insurance is verified, the clinic coordinator will collect it at this time if there is a copayment. 

The first visit with a provider is typically longer. We use this appointment to review your medical history and learn how to best help with your medical care.

What should I bring?

When you first arrive at our office, please check-in at the front desk and bring the following items with you:

  • New patient information form
  • Insurance card
  • Driver’s license or photo ID
  • List of medications you are currently taking
  • Recent labs (within the last year)
  • Imaging (most recent reports: colonoscopies, mammograms, chest X-rays, etc.)
  • List of all providers you have seen in the last five years
    • We need specialists’ names and phone numbers so we can request records.

Directions

From Interstate 530 S: Head south on I-530 S. Take exit 32 toward AR-256/White Hall. Turn left onto W. Holland Ave. Turn right to stay on W. Holland Ave. Turn right onto Dollarway Rd. Your destination will be on the right.

From US-79 N: Head northeast on US-79 N toward AR-54 E. Turn left to merge onto I-530 N. toward Little Rock. Take exit 36 to merge onto Princeton Pike. Continue onto W. Barraque St. Turn left onto Bryant St. Take a slight left onto AR-365 N/Dollarway Rd. Your destination will be on the left.

From Interstate 530 N: Head west on I-530 N. Take exit 36 to merge onto Princeton Pike. Continue onto W. Barraque St. Turn left onto Bryant St. Take a slight left onto AR-365 N/Dollarway Rd. Your destination will be on the left.
