We are Baptist Health Family Clinic-Conway West: a group of board-certified physicians and medical professionals who have the skills and experience to offer our patients high-quality care. As a family medicine clinic, we listen to our patients and work together to address their healthcare needs. 

Baptist Health Family Clinic in west Conway is a comprehensive medical facility that treats acute injuries and illnesses, such as strep throat, to chronic conditions, such as diabetes or asthma. Prevention plays a significant role in our family medical services and includes physicals, immunizations, health assessments, and more.

Patient Information

Once you have made your initial appointment, please read through the information below to help streamline your first visit with us.

Established patients may sign in to our MyChart patient portal to schedule an appointment, request an appointment, or cancel an upcoming visit with their primary care provider. Before your scheduled visit with your provider, you can use eCheck-In to update insurance information, medications, allergies, fill out questionnaires, and sign documents.

What can I expect at my first visit?

Our office is located in the Medical Office Building directly behind Baptist Health Medical Center in Conway.

Our patients are checked in and quickly taken to an examination room by a nurse to begin the information-gathering process.  A nurse will then perform an initial assessment before a healthcare provider spends as much time as needed with the patient to address any critical issues.

The first visit with a provider is typically longer. We use this appointment to review your medical history and learn how to best help with your medical care.

What should I bring?

When you first arrive at our office, please check-in at the front desk and bring the following items with you:

  • Recent labs (within the last year)
  • Imaging (most recent reports: colonoscopies, mammograms, chest X-rays, etc.)
  • List of all providers you have seen in the last five years
    • We need specialists’ names and phone numbers so we can request records. 
  • List of medications you are currently taking
  • Insurance card
  • Driver’s license or photo ID
  • New patient information form

From Interstate 40 W: Merge onto I-40 W. Take exit 129A-B for US-65 BUS W toward AR-60 W. Merge onto US-65 BUS N/E Dave Ward Dr. Turn left onto Exchange Ave. Turn right.  Your destination will be on the left.

From Interstate 40 E: Head south on I-40 E. Take exit 129 for US-65 BUS/AR-286/Dave Ward Drive W toward AR-60. Use the 2nd from the right lane to turn right onto AR-286 W/E Dave Ward Dr. Turn left onto Exchange Ave. Turn right. Your destination will be on the left.
