BAPTIST HEALTH Rehabilitation Institute is an Acute Rehabilitation Facility, not a Long Term Care Facility. Your return to your community is our mutual goal. You, your family members, your physician and your treatment team members will all be involved to improve the likelihood of a successful transition. Your health and your resources are different from anyone else. For many people, discharge is as simple as packing up and going home. Others may not be ready to go home and may require more treatment in a different care setting, such as a Recuperative Care Facility.

Your care coordinator will assist you and your family with areas such as:

• Living and Care Arrangements
  
• Home Health Nursing and Therapy
  
• Outpatient Therapy
  
• Exploring financial and other assistance resources available in your community

Your therapists will counsel and advise you regarding necessary equipment which can improve your ability to be independent. In most cases, your equipment will be here or delivered to your home at the time of discharge.
