Please list the team captain for each team and his/her contact information. Also list the names of the players on each team, and indicate the preference for course and tee time.

The tee time choices are Monday AM, Monday PM, Tuesday AM, Tuesday PM. The two courses are Founders and Bear Den Mountain. *Note: All Diamond and Platinum Sponsors have exclusive right to play on the Bear Den Mountain Course on Monday or Tuesday, but MAY choose to play on the Founders Course.
